Physical Therapy
- Helps patient improve in life activities such as bathing, dressing, and basic household activities.
- Recommends adaptations and home modification to make the home safer.
- Establish a daily routine that allows you to do daily tasks without overtaxing.
Occupational Therapy
- Improve ability to effectivly communicate.
- Assist with memory problems.
- Restore ability to swallow safely.
Speech Language Pathology
- Assistance in community resources education for financial, social and/or transportation issues.
- Helps establish long term plans, crisis intervention and family counseling.
Medical Social Work
- Registered Dietitian assists with nutrinal supplimentation and meal planning in cordination with specific instructions by your physician for disease management and/or suggestions by the Speech Therapist.
Nutritional Services
- Help getting in and out of bed.
- Help walking, bathing, toileting, and dressing.
Home Health Aides
- Our clinicians work with our Medical Social Workers to obtain resources to assist our patients in financial, social and/or transportation issues.
- They help establish long term plans and family education.
Community Resource Education
- Rehab At Home, Inc. has on-call telephone support 24 hours a day. We are open from 8:30am to 5:00pm Monday through Friday.
